The practical answer
Count the actual Forms 1042-S attached to one permitted paper group. Total their box 2 amounts for T line 2 and their boxes 10 plus 11 for the applicable T line 3. Do not use a recipient count, page count or unrelated annual ledger total.
This guide uses the 2025 final 1042-T, the current final T edition when checked. Its example is for tax year 2025 and assumes paper eligibility and statement grouping have already been reviewed. Use the applicable final edition for another year.
Freeze the attachment manifest before calculating totals
Create an internal manifest of the exact Copy A statements intended for one transmittal. Record each unique form reference, version, withholding agent, year and reviewed group. Keep recipient names or internal references sufficient to identify the form without exposing sensitive identifiers unnecessarily.
Separate original and amended statement populations and apply the other required grouping dimensions. A manifest combining different paper types cannot be fixed merely by calculating an accurate overall sum.
Mark the manifest version and compare it with the printed attachments. A calculation based on an earlier export can remain mathematically correct while no longer describing the packet waiting to be mailed.
Map the instructed source fields exactly
The 1042-T instructions specify the count and amount sources. Keep those columns in the workbook even if the accounting system uses different labels.
| T entry | Source | Control |
|---|---|---|
| Line 1d | Number of attached paper S forms | Count records in the group, not recipients or pages |
| Line 2 | Sum of attached S box 2 | Use the gross-income fields actually being transmitted |
| Line 3a or 3b | Total attached S boxes 10 plus 11 | Use the applicable chapter line, not both |
| Line 1a | Chapter of all attached statements | Match the group rather than inferring from tax amount |
Do not add box 7a withholding again on top of box 10 because a report exposes both. The transmittal instruction identifies the fields to total, and including overlapping source fields would distort that comparison.
Count statements rather than recipients or print pages
Count each actual S form in the group once. Two reviewed statements for the same recipient can still be two separate forms. Conversely, extra recipient copies or a second printout of one Copy A should not increase the attachment count.
Keep duplicate printouts out of the envelope and retain their disposition in the preparation notes if needed. If a statement is moved to another group, remove it from this manifest and include it in the destination group before recomputing either cover sheet.
Use a physical recount after assembly. The manifest provides the expected count; the assembled Copy A set provides the actual count. Compare them explicitly rather than assuming that a successful print command produced every intended attachment exactly once.
Worked example: include box 11 in the transmittal tax total
Fictional 2025 example. Assume three final statements belong in one permitted chapter 3 paper group and their underlying values have already been reviewed. The workbook copies the following fields without independently calculating tax rates.
| Internal form | Box 2 | Box 10 | Box 11 | Boxes 10 plus 11 |
|---|---|---|---|---|
| R-01 | $1,000 | $100 | $0 | $100 |
| R-02 | $2,000 | $200 | $25 | $225 |
| R-03 | $500 | $0 | $0 | $0 |
| Totals | $3,500 | $300 | $25 | $325 |
T line 1d is 3, line 2 is $3,500, and line 3b is $325. The tax bridge is $300 + $25 = $325, so copying only the $300 box 10 subtotal would understate this transmittal by $25.
Line 3a is not completed for this chapter 3 group. A bank deposit or annual tax-payment ledger is not substituted for the specified $325 total, because the transmittal describes these attached forms.
Investigate count and amount differences in a useful order
Start with membership: missing forms, duplicate copies, wrong versions and statements assigned to another group. Then compare field mapping and signs. Finally inspect individual records whose amounts differ from the source export.
Keep a difference column for count, gross income and the combined tax total. A zero gross-income difference does not prove the tax columns agree, and matching dollars does not prove the attachment count is correct.
If a source statement is wrong, return it for the appropriate underlying review and correction process. Do not change the transmittal alone to force agreement with an outside ledger while leaving the attachments inconsistent. Refresh the manifest and cover sheet after the source change is approved.
Retain the exact reconciliation that supported the mailed packet
Save the manifest, source copies, calculation and completed T as one versioned packet. Record the actual reviewer and review result only after that work occurs. If printing or assembly changes the contents, repeat the affected count and totals checks.
The 2025 T instructions say that use of the transmittal does not change the separate Form 1042 obligation. They also address amended Form 1042 when an attached S changes specified amounts on a previously filed return. Keep that annual-return review separate from the packet arithmetic.
The download provides the field map, row calculations and final comparison. It supports reproducible evidence of what this cover sheet summarizes, without representing the checklist as proof of mailing or IRS acceptance.
Reconcile the cover sheet to the actual paper attachments
Read the workflow as text
- Freeze one permitted group. Identify current Copy A records and their manifest version.
- Count forms once. Separate form count from recipients, pages and extra copies.
- Total the specified fields. Sum S box 2 and S boxes 10 plus 11.
- Compare the assembled packet. Retain the matching cover, attachments and reconciliation.

Does line 1d count recipients?
It counts attached paper S forms. A recipient may have multiple reviewed forms, while extra copies do not create additional records.
Which amounts support T line 2?
The sum of gross-income amounts in box 2 of the S forms attached to that transmittal.
Is the tax total only the sum of S box 10?
No. The 2025 T instructions specify the total of S boxes 10 and 11. Preserve both columns in the reconciliation.
Can I complete both chapter tax lines on one T?
No. Complete the applicable chapter 4 line 3a or chapter 3 line 3b for the group, not both.
What if the ledger and attachments disagree?
Investigate the underlying source records and grouping. Do not change the cover sheet alone to an outside total while leaving inconsistent attachments.
