The practical answer
Read 1042-T as a field map for one paper statement group: agent identity, type indicators, attachment count, gross-income total, chapter-specific tax total and signature. Each entry needs a source in the reviewed records behind that packet.
The example below redraws the information relationships of the 2025 final 1042-T, the current final T edition when checked. It is a fictional learning example, not a completed IRS form. Use the official form for actual permitted paper filing.
Establish what this example assumes
The IRS about page identifies T as the form used to transmit paper Forms 1042-S. Do not create a T for an electronic S submission. This example assumes the withholding agent's filing-method review permits paper.
It also assumes the underlying S records, their chapter and status codes, and the required grouping have been reviewed. The annotations explain where values come from; they do not determine treaty eligibility, withholding rates or recipient classification.
For another reporting year, obtain the applicable final form and instructions before using the field map. The visible example year is 2025, even though these sources were checked in 2026. Do not relabel an old form by changing only its heading.
Annotate agent identity and the statement type
| Form area | Source record | Review note |
|---|---|---|
| Withholding-agent name, address and EIN | Verified agent record and Forms 1042/1042-S | Identity must match the related forms |
| Chapter 4 and chapter 3 status codes | Reviewed agent classifications | Both codes are required at the top |
| Line 1a | Attached S chapter indicator | Enter one chapter, 4 or 3 |
| Line 1b | Current statement type | Choose original or amended |
| Lines 1c, 1e and 1f | Applicable reviewed S facts and special rules | Mark only when the specific instruction applies |
| Line 1d | Final Copy A manifest | Count attached S forms |
The form instructions distinguish the agent's two status codes from the single chapter indicator on line 1a. Completing a chapter 4 group does not mean omitting the agent's chapter 3 status code.
Annotate amounts, FINAL and the signature area
Line 2 comes from the attached S box 2 gross-income amounts. Line 3 comes from the total of attached S boxes 10 plus 11. Use line 3a for the chapter 4 group or line 3b for the chapter 3 group, and do not complete both.
The FINAL indicator is a separate factual determination about additional S filing requirements for the year, including amended and electronic forms under the 2025 instructions. It is not simply a marker that the team has reached the end of today's print queue.
The signature area contains signature, title, date and daytime phone fields. The appropriate signer must review the completed transmittal and accompanying documents. A training worksheet or typed reviewer name should not be treated as an actual signature or completed approval.
Worked example: two chapter 4 statements support one cover sheet
Fictional 2025 example. Cedar Reporting Entity has two reviewed, original, non-pro-rata chapter 4 statements in one permitted paper group. Neither special line 1e nor line 1f applies. The sample does not supply real taxpayer identifiers or a signature.
| Internal S record | Box 2 gross income | Box 10 | Box 11 |
|---|---|---|---|
| A-01 | $4,000 | $300 | $20 |
| A-02 | $6,000 | $450 | $30 |
| Total | $10,000 | $750 | $50 |
The field map gives line 1a = 4, line 1b = original and line 1d = 2. Line 2 is $4,000 + $6,000 = $10,000. Line 3a is $750 + $50 = $800. Line 3b is not completed.
The $800 is a sum of supplied, reviewed statement fields, not a withholding-rate calculation. The agent's identity and both status codes must come from its actual reviewed master record. The FINAL and signature fields require their own factual review before an actual form could be completed.
Test the map against the assembled packet
Take each completed T field and point to its source. The count should point to the manifest and actual attachments, the amounts to the specified S columns, and the agent identity to the related forms. A field with no source reference needs investigation.
Check that the two statements in the example still match the current manifest after any revisions. If one is replaced, moved to another type or removed, refresh the corresponding count and amounts. A previously correct cover sheet may no longer describe the new packet.
Inspect the physical output for legibility and completeness. Follow the official instruction to return the entire transmittal page, with the proper Copy A attachments. Keep the learning diagram separate from the actual IRS form sent.
Save the review map with the official completed documents
Retain the worksheet, official T copy, S copies, manifest and actual review evidence together. The diagram can explain the source relationships, while the official completed form preserves what the team actually used.
The 2025 T instructions also state that T does not replace the separate Form 1042 obligation and identify when attached S changes require an amended Form 1042. Keep that review in the record if applicable, rather than assuming that a cover-sheet total updates the annual return.
The download gives a field-by-field source index for repeating this exercise with your own permitted paper packet. It helps a reviewer locate support quickly without presenting the fictional example as a filing-ready document.

Read the workflow as text
- Agent identity and status codes. Use matching 1042/1042-S agent facts and both chapter status codes.
- Line 1: type and count. Read the reviewed chapter/type flags and count actual Copy A forms.
- Line 2: gross income. Total box 2 from the attached S statements.
- Line 3: applicable chapter tax. Total S boxes 10 plus 11 on line 3a or 3b, not both.
- FINAL and signature review. Review additional filing requirements and complete actual signature fields.

Can I send the diagram instead of the official T?
No. It is a learning map of field relationships. Use the official form and applicable instructions for an actual permitted paper submission.
Are the top status codes the same as line 1a?
No. The agent has chapter 4 and chapter 3 status codes at the top. Line 1a identifies the single chapter for the attached group.
Does a chapter 4 group omit the chapter 3 agent status code?
No. The 2025 T instructions require both withholding-agent status codes regardless of the payment type.
Does the example calculate a withholding rate?
No. It totals supplied S values using the T instructions. The underlying tax and classification calculations must already be reviewed.
Can I change 2025 to another year on this example?
Obtain the applicable final form and instructions for the actual reporting year. The example is explicitly based on the 2025 final edition.
